ILMA Meeting Registration Cancellation Request Form

All cancellation requests must be submitted in writing. Cancellations before Friday, September 18 will be granted a refund less a $225 administrative fee to cover credit card processing fees and other expenses related to registration. Cancellations requested on Friday, September 18 or later will not be refunded except for documented medical issues and will be considered on a case-by-case basis. A $350 cancellation fee will apply.

The form below must be submitted for cancellations requested after Friday, September 18.
